If rolling over in bed, looking up at a cabinet, or tipping your head back at the sink sends the room spinning for a few seconds — that’s the signature of BPPV (benign paroxysmal positional vertigo). It’s the most common cause of vertigo in adults over 55, and here’s the part worth celebrating: it’s one of the most fixable conditions in all of healthcare.


What’s Actually Happening

Your inner ear contains tiny calcium crystals that help sense gravity. In BPPV, some crystals break loose and drift into one of the ear’s semicircular canals, where they don’t belong. When you change head position, the crystals tumble, sending a false “spinning” signal to your brain. The spinning is brief and intense — usually under a minute — and is triggered by specific positions.


The Fix: Repositioning, Not Medication

Dizziness medications like meclizine only dull the symptom — they can’t move the crystals. The actual treatment is a canalith repositioning maneuver: a precise sequence of head positions that guides the crystals out of the canal and back where they belong. The best known is the Epley maneuver, but the correct maneuver depends on which canal is involved and which type of BPPV you have — which is exactly what our positional testing determines.


Most patients with BPPV improve dramatically in 1–3 visits. Many feel the difference before they leave the first appointment.
